He mentioned nothing in his letter about Sarah's forced operation, the terror it still held for her, the serious blood loss she had sustained, the postoperative crisis, the pneumonia and lung collapse or, finally, her startling weight loss and malnutrition - all in the space of two weeks. He told the department nothing about the parents being so alarmed by those developments that they didn't think their daughter was fit for chemotherapy. However, he did acknowledge Sarah's emotional distress since the operation, which he maintained was the parents' fault rather than the effects of a forced surgery and its serious complications. He wrote:

I have a deep concern that the psychological ill-effects of their [the parents'] continued presence is having on Sarah [sic]...I believe it is extremely important that Sarah have a series of formal psychological assessments to ascertain her views of what has happened and what her views of death and dying are if she does not have any further therapy.
